• Acord Commercial Insurance Application
• Acord Workers Compensation Application
• WCT Supplemental Workers' Compensation & Auto Application
• Currently valued Loss Runs (5 years + current)
• Current Experience Modification Worksheet, if applicable
• Indicate If quote is to include commission or to be quoted on a net basis
• Indicate if account is to be direct bill or “Pay As You Go”
  
• The following types of businesses require supplemental applications to be completed by clicking title:
  • Emergency Medical Services 
  • Child Daycare
  • Home Care  
  • Housing Authorities
  • Long Term Care & Assisted Living 
  • Medical Services 
  • Museums 
  • Non-Emergency & Public Transport 
  • Schools 

SPECIAL PAYROLL FEATURES - PLEASE ADJUST PAYROLLS ACCORDINGLY

• Clerical payroll (8810) must be broken out separately regardless of governing code(s) used
• The Trust caps each individual's payroll at a maximum of $75,000 per year including Executive Officers, Owners and Members of an LLC
